How to prove your COVID-19 Vaccination Status

QCG is adhering to the Queensland Government directive to minimise the risk of COVID-19 infection for our patients and staff.

We require all patient’s to provide their COVID-19 vaccination status when booking an appointment or procedure. To be considered fully vaccinated you must prove 2 shots of Comirnaty (Pfizer) or Vaxzevria (AstraZeneca).

There are a variety of methods to prove you are fully vaccinated:

  1. This can be done by uploading your COVID-19 Digital Certificate (PDF file you sourced from Medicare) when submitting our Appointment request or New Patient Registration webforms.
  2. If you are unable to upload your vaccination status with Medicare's PDF (via your my-gov account), you can also demonstrate this using the QLD Government’s Check In Qld App (setup to show a green tick).

  3. If you are unable to prove your status with other methods, please bring a recent printout that includes the date and time from your my-gov account on arrival.

Communicating COVID-19 Symptoms

If you are recently or currently experiencing any of the following symptoms, please notify our admin staff before presenting to the practice rooms:

  • Fever greater than 37.5°C.
  • Difficulty breathing.
  • Cough.
  • Sore Throat.
  • Fatigue.

Our Requirements for unvaccinated patients

Unvaccinated patients will be required to wear a mask at all times. No unvaccinated support visitors will be admitted to our rooms. For those entering via a hospital campus they will be refused entry at the hospital entry point.

We may request a negative covid test prior to attendance if deemed high risk.

Our Requirements for Vaccinated patients

If you are fully vaccinated, but are also recently or currently experiencing COVID-19 Symptoms (listed above) please notify our admin staff before presenting to the practice rooms.
